Individual Supports
Individual Supports are services that provide direct support and assistance for consumers, with or without, the caregiver present; in or out of the consumer’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion into the community, as outlined in his/her Service Plan. Individual Supports may include, but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.
Community Based Supports
Beacon – New Jersey provides one-on-one direct support for community-based activities, with a caring staff offering training and supervision as our consumers learn and perform basic self-care, social skills and activities of daily living. This may involve attending events, ordering off a menu at a restaurant, learning to cook and do laundry, and more, either in or out of the person’s home.
Behavioral Supports
Beacon – New Jersey works closely with individuals and, as appropriate, their families to address behavior through the development of behavior support plans. We conduct a thorough assessment, then develop and implement a structured plan, offering ongoing training and supervision of caregivers and staff as well as continuous monitoring and periodic reassessment of the plan.
Transportation
Beacon – New Jersey provides the individuals we support with transportation so they can access their surrounding community and partake in all the area has to offer. Our reliable transportation allows our individuals to gain access to services, activities and resources — whether medical appointments, shopping for groceries or personal items, visiting with friends, going to dinner or a movie, and more.
Representative Payee
Families and guardians are offered the option to have Beacon – New Jersey serve as representative payee, appointed by the U.S. Social Security Administration to receive Social Security and/or SSI benefits for someone who cannot manage his or her finances.
