Most individuals with complex IDD and behavioral health needs follow a costly cycle: a triggering event sends them to acute care, they stabilize, get discharged — and 21% are readmitted within 90 days. Traditional providers weren’t built to absorb this population.
Beacon fills that gap. Purpose-built for the highest-acuity discharges other providers can’t support, Beacon reduces readmission risk, relieves inpatient capacity, and delivers measurable cost savings to payers and families.

Navigating high acuity behaviors, chronic illness, and mental health disabilities requires effective “care team” coordination, communication and collaboration.
Our care teams consist of direct support professionals, licensed clinicians, social workers, behavioral therapists and nurses that work together to manage the “whole person.” We coordinate directly with primary and specialty care providers to ensure seamless community care.
Quality and safety is accomplished by having an integrated and single-source health management enterprise to monitor treatment plans, track goals and gaps, trend impact and ensure accountability, quality and continuity of care.
Beacon is the first national provider to partner with the National Association of Direct Service Professionals (NADSP). Beacon transforms “jobs” into “careers” and we require and invest in the national certification of all staff, ensuring each person served is supported by a credentialed professional, not just a caregiver.
We’re the first in the industry to provide a dedicated lifeline for frontline staff. StarTeam is our internal call center exclusively dedicated to support in-home staff with on-the-spot clinical questions, policies, training, systems support or help managing an escalated situation. Since 1964, Beacon Specialized Living has delivered compassionate care to individuals with developmental disabilities, autism, mental illness and/or severe chronic conditions. Beacon creates new models of care that encompass safe housing, healthy food and affordable transportation, and we prioritize “connectedness” with peers, caregivers, and local communities. These needs are essential to achieving meaningful improvements in function, independence and well-being. Through community-based coordinators with local expertise, we support individuals in person, gaining their trust and developing personalized goals to address their physical, mental health and social needs.
We achieve sustainable outcomes by embracing or developing best practices for our specialized services. We apply evidence-based care via a network of therapists, clinicians and nurses, effectively supporting the “whole person” and helping them reach their highest level of independence possible. We invest significantly more in people and infrastructure than any other provider, and demonstrate that our community-based services are far more humane and effective in achieving sustainable improvements in behaviors, health outcomes and quality of life.
